American Vision Store Policies

  • Once an order has been placed it cannot be canceled.
  • No refunds, store credit only.
  • Any adjustments made to patient's glasses are at their own risk.
  • American Vision cannot be held responsible for any damage made to patient supplied frames when replacing lenses.
  • All of our frames are warranted for one year (from the date of purchase) against normal wear and tear.
    • There will be a charge of $29.00 for all warranty replacements.

American Vision Doctor Policies

  • All examinations or copays must be paid for on the date of service.
  • Any vision insurance or coverage must be presented at the time services are rendered. We cannot bill insurance after the date of service.
  • No show or Short notice cancellations:
    • If you need to reschedule or cancel your appointment, please do so with at least 24 hours notice. If this is not possible, call as soon as possible so that another patient can be given your appointment time. Please understand that American Vision of Lynbrook do not over book our schedule to cover for patients canceling at the last minute or not showing up. We reserve your appointment time for you specifically. If you cancel on short notice or do not show up, that is lost opportunity that another patient could have used to be treated and lost revenue to our practice. We understand that unanticipated events happen occasionally in everyone's life, but in our desire to be fair to all patients and maintain a viable practice, we have instated a $25.00 penalty fee for no-shows and/or short notice cancellations. Insurance will not cover this charge and this charge is in addition to any additional doctor fees you may incur. Repeated missed appointments may result in our doctors sending you a letter discharging you from the practice.
  • Out of respect and consideration for your doctor and other patients please plan accordingly and be on time for your appointment. Patients who are late for any appointment may be asked to reschedule at the doctor's discretion.
  • Once your contact lens fitting is in progress and you have been notified that your contact lenses are in, you must return for any follow up care within 30 days. If you are non-compliant and do not return within this time frame you will be billed an additional $39.00 for this continued care.
